PROOF OF IDENTITY and ACCESS TO RECORDS
You MUST
provide Proof of Identity documents if applying for certificates for births
or changes of name that happened less than 75 years ago, marriages less than
60 years ago and deaths less than 10 years ago.
A certificate
will NOT be issued until you send or bring the required Proof of
Identity documents to the Registry (GPO Box 4332
Melbourne 3001 or 589 Collins St
Melbourne) with
the receipt number you will obtain with your transaction.

Events that
are older than the periods stated above are not subject to the access policy
or proof of identity.
PROOF OF IDENTIFICATION DOCUMENTS
If you are
applying for a certificate requiring proof of identity you must provide 3
forms of identification as shown below. Copies of documents must be
certified by a member of the police force.

should provide one (1) form of identification from each list.
- If you are unable to
provide identification from List 1 you must provide two (2) forms of
identification from List 2 and one (1) form of identification from List 3.

POLICY ON ACCESS TO RECORDS
Persons
allowed to apply for certificates from the Registry under the Family
Relationship provisions are shown here. For the Registry's full Policy on

Birth and Change of Name Certificates
- The registered person.
- If the registered person
is aged less than 18 years: Parent, Non-parental legal custodian or
Guardian. Document establishing custody or guardianship to be produced.
Marriage Certificates
- One of the parties to the
marriage.
- Child of the marriage or
Child of the bride but not of the marriage. Must produce written
authority of one of the parties to the marriage, or apply through an
accredited Welfare Agency.
Death Certificates
- Domestic Partner, Parent
or Child. Evidence of relationship must be established.
- Non-parental legal
custodian or guardian. Document establishing custody or guardianship to be
produced.
- Other relatives,
Ex-partner. Must produce the written authority of next of kin, or evidence
that the certificate is needed to establish some legal right or
entitlement.
If the
certificate relates to a person other than yourself and you do not qualify
under the Registry's Access Policy you must provide the written consent or
authority from an authorized person and that person must also provide 3
forms of identification as shown below.
